Our team has just finished a template set for the CPSP (Comprehensive Perinatal Services Program) for a large multi-specialty group in Los Angeles. The CPSP program at our client's facility will be using our template set for their live production environment. The CPSP template is integrated smoothly into the current NextGen OBGYN templates and vise versa. Many of the CPSP templates are built to record the patients initial, second trimester, and third trimester visits with the data being copied forward each visit.
The CPSP template set has been built with 16 medical record templates such as Family Planning Counseling, Group Class Visit, Individual Care Plan, Master Template, PFFQ (PERINATAL FOOD FREQUENCY QUESTIONNAIRE), Postpartum, and Prenatal. We also integrated the use of System Templates in order for the users to quickly and easily maintain the list of referral sources, instead of having to call us up to make those changes.
Master Template: Overall summery of patient's OB history, problems and allergies.
Orientation Template: Patient checklist of education and other handouts.
Prenatal Template: This is used as a assessment and reassessment tool. Also contains info for personal information, pregnancy care, nutritional assessment summary, economic resources, coping skills, housing, educational interests, PHQ-9 Questionaire, transportation, nutrition, DV screening, current health practices, dietry, and OB Education.
Postpartum Assessment: ICA intervention, anthropometric, psycho-social, EPDS, OB education, clinical - outcome of Pregnancy, health ed, nutrition.
ICP Template: This template covers in detail the first, second and thrid assessments - identified problems and risks, plus postpartum.
